Medical Declaration Forms

This form MUST be completed by anyone that is representing Great Britain or their Home Country internationally or competing in a British Swimming, ASA, SASA or WASA National event (all disciplines, excluding masters) and returned to the ASA Membership Department at the address below.  A new form MUST be completed annually even if the medication prescribed has not been altered or if no medication is being taken and whenever the medication is changed.

Important Information for 2008

Coach Passes

Any coaches wishing to be on poolside with athletes at this years competition in December all need to be CRB checked with the ASA.

This is very important as coaches without the relevant CRB will not be issued with a coaches pass and therefore will not be allowed on poolside.

NO EXCEPTIONS WILL BE MADE

Skill Assessments

Please ensure all applications are submitted in bulk by clubs and not separately by individual swimmers. All clubs are to provide sufficient, appropriately qualified assessors to accompany their swimmers.  

0 - 3   swimmers        no assessor will be required
3 - 5  swimmers          1 assessor will be required, 
5 -15 swimmers          2 assessors will be required
15 -25 swimmers        3 assessors and this will increase in multiples of ten.

Skill Assessors Re-Training and Examination

All current Basic and Advanced Skill Assessors must take an Assessors examination before 31 December 2008 if they wish to continue as an Assessor in 2009.  Re-Training and Examination days are being arranged. Further details will follow.
